Completing the Pre-Printed FedEx Label:

  1. Fill in the From section, including your phone number.
  2. Tear the card along the center perforation and peel off the back of the form and attach it to the top of your shipment.
  3. Save the left side of the form for your records.
  4. Call us at 800-325-3056 to schedule your FedEx pick-up.

  1. Section 1: Fill in your name and address
  2. Section 3: Fill in Keller’s name and address
  3. Section 4a: Check the appropriate box for the type of service required
  4. Section 5: Check the Other box
  5. Section 6: Check the No box
  6. Section 7: Check the Recipient box, Enter Keller’s FedEx Acct. Number (call us for our account number), Enter Total Packages, Total Weight of 1pound and Total Declared Value of $100 per package
  7. Click here or call Keller at 1-800-325-3056 to schedule your pick-up.
