Obstructive Sleep Apnea Treatment


Keller offers two different oral appliances for the treatment of Obstructive Sleep Apnea: The Thornton Adjustable Positioner (TAP®) and the Elastic Mandibular Advancement (EMA®). Both the TAP and the EMA feature the ability to adjust the amount of mandibular advancement, opening the patient's airway.

Here are some Tips for medical insurance reimbursement:

  1. Obtain a copy of the PSG (sleep study) results with a diagnosis of Obstructive Sleep Apnea (OSA)
  2. Letter of medical necessity
  3. Pre-Authorization from patient's insurance company may be required. Always call to determine necessity for Pre-Auth.
  4. SOAP Narrative
  5. Description of TAP
  6. Description of EMA
  7. Informed Consent Form
  8. Referral letter signed by physician
  9. Completed CMS 1500 medical claim form. CMS forms can be purchased online at http://www.cms.hhs.gov/ or google "CMS 1500" for a variety of vendors.
